Affidavit for passport of child in India
One of the parent who is applying for the passport of their child in India needs to apply for an affidavit. The following needs to be submitted at the ITA:
i.Duly filled-in form (Form F) 
ii.Photocopy of the Birth certificate of the child.
iii.Photocopy of the first and last page of the passport of both, father and mother. (Original passport to be shown at time of submission).
iv.Photocopy of ARC (Original to be shown at time of submission).
v.In case of divorce of parents of the child, the Court Custody papers, attested by the Notary Public.
vi.In case of a single parent, declaration of the parent shall be attested by a Notary Public.
