Birth Registration
The birth of any child in Taiwan needs to be registered at ITA before applying for passport for new-born. The following needs to be submitted at the ITA:
i.Duly filled-in form (form B).
ii. Original and photocopy of the Birth certificate of the child issued by the Taiwan authorities.
iii.Photocopy of the first and last page of the passport of both, father and mother. (Original passport to be shown at time of submission).
iv.Photocopy of ARC (Original to be shown at time of submission).
v.Photocopy of Marriage Certificate of parents.
vi.In case of divorce of parents of the child, the Court Custody papers, attested by the Notary Public.
vii.In case of a single parent, declaration of the parent shall be attested by a Notary Public.
