Life Certificate to Pensioners
1.The following documents are needed:
i. Duly filled-in form (form G) 
.
ii.Covering letter explaining clearly the details of the pension account.
iii.Copy of the first page of Pension Account book showing the name and Pension Account number.
iv.Copy of the first and last page of the Indian passport. (Original passport to be shown at time of submission)
v.Photocopy of ARC (Original to be shown at time of submission).

2. The applicant should physically present oneself in front of the officer who shall be issuing the Life Certificate.
